Punctuality is important in order to maximize treatment time. It is best to arrive 5 to 10 minutes early in order to be relaxed for your appointment.    Out of consideration for others, please refrain from smoking in the office. Also, please turn off your cell phone and pager so that your treatment may be uninterrupted.    Please notify me at least 24 hours in advance of any need to change a scheduled appointment. Otherwise, you will be charged for the missed appointment, unless you are in an emergency situation.    Many insurance companies are now covering acupuncture treatment. and limitations in coverage. Please discuss these items with me. Superbills or Insurance Claim Forms will be provided upon request. I am a provider for Blue Cross/Blue Shield and for Kaiser Permanente.    Unless other arrangements have been made in advance, payment is expected on the day of treatment.
